- dates of employment
- last position held (job title at the time of leaving)
Please ensure you supply the National Insurance Number, as we will not be able to supply a reference without this.
Please note that if the individual worked for Capita via an agency or as an independent contractor, we are unable to supply a reference of employment and all requests should be directed to that agency they worked for.
